You must submit a non-returnable, clear, color picture of the dog with the nomination form,  and a 500 word or less description  of how the dog has demonstrated outstanding behavior to accompany your entry.  The reason for the nomination should have occurred between January 1, 2009 and October 31, 2009.  .  Your description may be typed or hand written.

The information and picture should be mailed to 3947 N. 19^th Street, Coeur d’Alene, ID 83815,  and postmarked no later than   October 31, 2009
